Togo Health started as a collaborative project to change the way healthcare is delivered. We've incorporated strategies from Ghana's mobile healthcare, award winning research projects, and in depth analysis of America's Value Based Care programs and post acute care systems.​

Primary Care Wherever You Are
At Home
Access to quality care is very important to our team. We send highly trained medical professionals to your home in order to provide primary care visits wherever you are. We also utilize telehealth for those tech savvy people looking for a more convenient way to meet with their provider.
Assisted Livings
If you have a loved one who has moved into an Assisted Living for some extra help with daily activities and healthcare assistance. We will gladly see them in the Assisted Living on a regular basis to ensure their needs are met.
Skilled Nursing Facilities
We care for patients while in Skilled Nursing Facilities both in long term care and short term rehab. We'll do everything we can to prepare you for success, including follow you at home after a transition of care.
